Speakers will describe what community health workers/promotores (CHWs/Ps) are, and will share best practices for integrating these patient advocates into the care team. Attendees will learn about practical and impactful approaches that have been successfully used to better manage multiple chronic diseases.
Ms. Rodriguez will describe the role of CHW/Ps in service delivery, healthcare research and clinical interventions. She will also provide an update on the progress of health disparities research and on interventions incorporating CHW/Ps to address social determinants of health. Ms. Meza will describe the role of CHW/Ps as true members of the health care team and will highlight cases supporting patients and their families.
